Mr. Otis Fields, Jr., age 94, of Mayfield passed away on Sunday, June 27, 2021 at Mills Health & Rehab in Mayfield, KY.
He was a member of Emmanuel Baptist Church in Mayfield, KY and a member of Laborers Union Local 1214.
Mr. Fields is survived by one son, Duane (LaVeda) Fields of Mayfield, KY; one daughter, Pam Wilkins of Mayfield, KY; three grandchildren, Laura Wilkins, Scott Wilkins and John Mark Fields; and three great grandchildren, Alivia, Archer and Max.
He was preceded in death by his wife, Geraldine Wilson Fields and his parents, Henry M. and Lucy Slayden Green.
Graveside services for Mr. Otis Fields, Jr. will be 3:00 p.m. on Wednesday, June 30, 2021 at Chapel Hill Cemetery. Rev. Duane Fields will officiate. Interment will follow.
Visitation will be 1:00 – 2:30 p.m. on Wednesday at Byrn Funeral Home in Mayfield, KY.
