COST: $15 PER STUDENT (Checks payable to Marshall County Health Dept.)
Forms need to be filled out and signed by a parent and athlete BEFORE the physical is completed. Forms are available online at KHSAA, MCHS, your child’s school or by clicking the links below. Middle school students will need a middle school form and high school students will need the high school form. If an athlete plays both high school and middle schools sports, they must complete BOTH forms.
This will be the ONLY physical date scheduled for the 2018-2019 school year. Please come at the appointed time for your grade level for the 2018-2019 school year. We will have several health care providers available for your convenience. You will be seen in the order you sign in.
Please enter the high school building at the freshman entrance of the south side of the building. This entrance is located near the Hwy. 68 entrance to the campus.

9:00 am – Sophomores
10:00 am – Juniors
11:00 am – Seniors
1:00 pm – 6th grade
2:30 pm – 7th grade
3:30 pm – 8th grade
